Chronic Disease Prevention

Chronic diseases - such as heart disease, cancer, and diabetes - are the leading causes of death and disability in the United States. These diseases account for 7 of every 10 deaths and affect the quality of life of 90 million Americans. Although common and costly, many chronic diseases are also preventable. Many chronic diseases are linked to lifestyle choices that are within your own hands to change. Eating nutritious foods, becoming more physically active and avoiding tobacco can help keep you from developing many of these diseases and conditions. And, even if you already have diabetes, heart disease, arthritis or another chronic condition, eating more healthful food and getting more exercise, whether it's a brisk walk, a bike ride, a jog or a swim, can help you better manage your illness, avoid complications and prolong your life.

Living Healthy- Chronic Disease Self Management Class

A highly interactive 6-session, 2.5 hour peer-led health education workshop that utilizes the evidence-based Chronic Disease Self-Management Program (CDSMP) curriculum developed and tested at Stanford University.

The Living Healthy workshop is for anyone dealing with a chronic or ongoing health condition and their caregivers who want to learn how to self-manage and take control in dealing with pain and fatigue, discover better nutrition and exercise choices, and learn better ways to talk with your doctor and family about your health.

Adults who have been diagnosed with a chronic disease such as arthritis, diabetes, heart disease, lung disease, stroke or other health problems, chronic mental illness or caregiver for someone with a chronic disease. 

Workshop are lead by a trained peer leaders who have or understand what its like to live with a chronic disease. 

The program covers topics such as tips to maintain and improve strength, flexibility and stamina, techniques to deal frustration, fatigue, pain and isolation, appropriate use of medications, tips to communicating effectively with family, friends and care givers, nutrition, how to evaluate new treatments and joy in learning that you are not alone! 

People that have participated in the program have had significant improvements in exercise, cognitive symptom management, communication with physicians, self reported general health, health distress, fatigue, disability and social/role activities limitations. They also spend fewer day in the hospital. Fewer outpatient visits and hospitalizations. Many of these results last as long as 3 years. 


Active Living Every Day Workshop

Regular physical activity is one of the most important things you can do for your health. It can help:

  • Control your weight
  • Reduce your risk of cardiovascular disease
  • Reduce your risk for type 2 diabetes and metabolic syndrome
  • Reduce your risk of some cancers
  • Strengthen your bones and muscles
  • Improve your mental health and mood
  • Improve your ability to do daily activities and prevent falls, if you're an older adult
  • Increase your chances of living longer 

Active Living Every Day is an evidence-based behavior change program that can help people increase your physical activity. The approach the workshop takes is unique because it addresses the root causes of inactivity rather than simply prescribing exercise. Participants learn the skills they need to become and stay physically active. These skills include:

  • Identifying and addressing barriers to physical activity
  • Increasing self-confidence about becoming physically active
  • Creating realistic goals and rewards for physical activity
  • Developing social support
  • Recovering from lapses in physical activity

The workshop meets for 1 hour weekly for 12 weeks, you get group support, a book describing the steps to the program, and pedometer to track your progress.

National Diabetes Prevention Program 

Classes teach you things you can do to prevent or postpone a diagnosis of Type 2 Diabetes.

  • Better food choices
  • Lose weight
  • Improve sleep
  • Get rid of stress
  • Increase physical activity

Weekly workshop for 4 months then monthly for 6 months with trained experts.

Program is approved by CDC and proven to prevent and/or delay the onset of Type 2 Diabetes. 

Who Should Attend?

  • Adults diagnosed with prediabetes, or at risk for developing diabetes.
  • Healthcare providers can refer any patient who has one or more of the following:
  • A history of gestational diabetes.
  • 2-Hour Plasma Glucose Test (OGTT) between 140 – 199 mg/dl.
  • Hemoglobin A1C between 5.7 – 6.4%.
  • Fasting Plasma Glucose (FPG) between 100 – 122 mg/dl.

  • Cuts students risk of developing diabetes in 1/2.
  • Modest behavior changes help students lose 5-7% of their body weight
